Onset and Progression

When did the pain start, and has it changed in nature or severity over time?

Have you noticed any triggers that exacerbate or alleviate the pain?

Pain Characteristics

Can you describe the pain as (sharp, dull, throbbing)?

Is the pain felt at all times or only at specific inactivity/recreation/working points?

Associated Symptoms

Have you observed any signs of inflammation in your legs?

Have there been any recent changes in weight or appetite?

Medication and Treatment History

Are you currently taking any medications other than for hypertension and hypercholesterolemia?

Did you use any treatments or medicines that would address or reduce this pain?

To get a broader perspective of his pain and how it affected him, the above questions were posed to Mr. Vin. This helps identify the problem's cause, the damage's extent, and the most effective course of action. It is essential to recognize the time of pain onset, pain characteristics, factors precipitating pain, functional consequences, symptoms accompanying pain, and past pain treatments to optimize pain management and enhance patient outcomes.

Parts of the Exam

Vital Signs

To check for hypertension, heart rate, and signs of systemic illness.

Peripheral Vascular/Extremities

Check for pulses, capillary refill, and peripheral artery disease (PAD) signs such as hair loss, colour changes, or ulcers.

Perform the Ankle-Brachial Index (ABI) to assess for PAD.

Cardiovascular

Auscultate the heart to check for murmurs, irregular rhythms, or other abnormalities.

Check for signs of heart failure or other cardiovascular issues that could contribute to leg pain.

Musculoskeletal

Diagnostic criteria: The inter-rater agreement is reasonable for site-specific assessment of flexibility, muscle power, and inactivity of malignant tumours or abscesses.

A simple touch should be used to feel for any signs of swelling, contour of leg abnormalities, and any lumps, as recommended by Wilson (2020)

Nervous System

Examining the patient’s reflexes, sensations, and strength is essential to rule out palsies and radiculopathies.

Skin

The signs of poor circulation include ulcer formation, mutative skin colour, or temperature variation of the limb.

Rationale

Such questions and examinations assist in determining what could be ailing Mr. Vin, likely the reason behind his leg pain. The patient has several attributes causing him/her to be at risk of developing PAD; hence, the cardiovascular and peripheral vascular systems must be assessed (Gul & Janzer, 2023). However, it also refers to the musculoskeletal and nervous system examination to identify the reason for the aches. It also helps him to obtain more understanding about his state, find out what other experts can say about his matter, and receive another examination result.
